We offer several ways of ordering using our blank order form:

Mail Order: When paying by check or money order, place your order using the U.S. Mail Order Form in Word 6.0 format or the HTML format (see below for the forms). Mail your order form and payment to:

Auctions: If you have won our eBay actions or purchased items from our eBay store, and prefer to pay by credit card using our secure shopping cart, please feel free to use our blank order form to pay for your auction. Just place the auction number, product description, final bid price, and quantity on and our shopping cart will do the rest. When there is a difference in shipping due to discount offered in the auction, we will refund any surplus.

When purchasing any of our items online or mail order, we accept credit cards, money order, personal checks or PayPal. When paying by PayPal, our ID is jcwood@sidell.com.
